How to Register for the Symposium

 

ON-LINE REGISTRATION is the only means of registering accepted by the Workshop.

All persons registering on-line by November 20, 2006 will receive an immediate on-line acknowledgement of registration followed by an e-mail, including user ID number and password to be used for further contacts or to revise your data (please do not forget to print it).

The deadline for on-line registration is November 20, 2006
After this date participants will have to register on-site in Milan.


Methods of Payment
The registration fees may be paid on-line in one of the following ways:

  Credit Card (American Express, Visa, Mastercard, and Maestro). Please enter the credit card number and expiration date in the appropriate box on the on-line registration form. Our on-line system is certified through "Verisign" and "SSL". If the credit card charge is successful you will be promptly notified by our on-line system. Please print the notification.

Participants must fax to:
Metod & Focus Group fax +39.02.57410138, a copy of the confirmation of the bank transfer together with the full contact details (name, affiliation, postal address, phone, fax, e-mail) of each of the attendee(s).

If a Company pays on your behalf through bank transfer or credit card, please ask the Company to clearly indicate your name, your full postal address (not the Company address), and your email address on the on-line registration form together with your name related to the payment. If complete contact information is not provided prior to the meeting, you will be asked to provide this information on-site before receiving your Workshop material.

Name Changes
After November 15, 2006 sponsors who wish to make name changes will be charged an handling fee of € 30,00 (VAT 20% included) per name change. A new revised registration form must be completed for each replacement participant before any name change will be made. Individuals can make name changes only in exceptional cases. Written permission from the original participant will be required in order to process an individual name change.

For Italian Participants Only
All the Italian participants with an IVA number must indicate such a number (and its corresponding address) on the on-line Registration Form (DPR 633/72).
The regular invoice of the registration will be sent only to those who have indicated the fiscal code and/or the IVA number (compulsory if available) of the actual payer.

Privacy Law
The personal data will be handled exclusively by the Organising Secretariat according to the law 675/96 and law decree 196/2003.

Registration Cancellations
Written notice of cancellation must be sent to Metod & Focus Group no later than November 15th.
If cancellation notice is received by this date, the registration fee will be refunded, less 30% for administrative charges. Refunds will be made after the meeting.
Cancellations received after November 15, 2006 will not be refunded: however, these registrants will receive their Symposium scientific material by mail after the meeting.

Shuttle service is organised from hotel to Istituto Nazionale dei Tumori.

How to make a hotel reservation

 

Make your reservation by October 31, 2006. After that date, rooms can not be guaranteed.
A voucher will be sent out as confirmation for your reservation.

Hotel Reservation Cancellation
